Extension Agent Reporting
Dear STEM Equity Pipeline Participant,
We want to thank you for your participation this last year in the many professional
development events we have been able to offer through the STEM Equity Pipeline project
due to the generosity of the National Science Foundation.
As you may recall the Five Step Program Improvement Process Training was intended
to equip you as trainers (extension agents) to share what you learned in a variety
of settings. These could have included one-on-one sharing with your colleagues,
conducting workshops at a local, state or national conference using materials from
a portion of the training, or even conducting the full training yourself with others.
As part of the evaluation of our work we need to know from you what you have done
with the training you have received. If you have used any of what you learned at
the Five Step Program Improvement Process Training please complete this
simple survey telling us about what you did, where you did it and
whom you did it with. Please complete a survey for each time you have used the information
you learned since you participated in the training. The survey will also be available
for you to complete for any future training you will conduct. It is very important
to us that you let us know how you have used the training you received and give
us feedback about professional development and curriculum materialsK that could
be developed to help you in the future.
